Selektive hypophysäre Schilddrüsenhormonresistenz

Die selektive hypophysäre Schilddrüsenhormonresistenz betrifft nur den Rezeptor in der Spleißvariante, die in Hypothalamus und Hypophyse exprimiert wird. Die Resistenz der Hypophyse gegenüber dem Schilddrüsenhormon verhindert eine Rückkopplung und führt damit zu einer Überstimmulation der Schilddrüse. Klinisch findet sich eine erhöhte Schilddrüsenhormonsekretion mit Struma und allen Symptomen einer Hyperthyreose. Das TRH und das TSH werden meist unangemessen hoch gemessen. Ursächlich finden sich verschiedene Mutationen im Schilddrüsenhormonrezeptor (THRB). Dies scheint jedoch nicht die alleinige Ursache zu sein, weil nicht selten unterschiedliche Familienmitglieder mit dem selben Genotyp eine unterschiedliche Klinik aufweisen.
