Der Pseudohypoaldosteronismus Typ 2 ist wie jeder Hypoaldosteronismus durch metabolische Azidose und Hyperkaliämie gekennzeichnet. Daneben sind der Reninspiegel erniedrigt, der Aldosteronspiegel meist normal und der Blutdruck erhöht. Die Vererbung erfolgt autosomal dominant.
Die Vererbung is autosomal dominant. Neben den üblichen Symptomen wie metabolische Azidose und Hyperkaliämie ist trotz supprimierter Reninsekretion ein normaler oder gar erhöhter Aldosteronspiegel vorhanden. Der Blutdrck ist typischerweise erhöht. Das gute Ansprechen der Erkrankung auf Thiaziddiuretika und Aldosteronanaloga ist sowohl von diagnostischer, wie auch therapeutischer Relevanz.
Zur Diagnostik gehören Bestimmungen des Säure-Basenhaushaltes und der Elektrolyte (Na, K, Cl). Der Reninspiegel ist erniedrigt und das Aldosteron meist normal. Die Ansprechbarkeit auf Aldosteron bleibt erhalten. Pathognomonisch ist zudem das gute Ansprechen auf Thiaziddiuretika.
Mutationen der WNK Serin-Threonin Kinase rufen diese Erkrankung hervor. In welcher Funktionstörungen diser Proteine zu den pathologischen Veränderungen führen ist noch nicht im Detail bekannt. Die Kinasen werden in der Pars convoluta distalis (DCT) und den kortikalen Sammelrohren (CCD) gefunden. Hier sowohl intra- als auch extrazelluär, in den tight junctions.Neben den beiden gut charakterisierten Genen WNK1 und WNK4 gibt es noch ein bisher nicht näher eingegrenzten Lokus auf dem Chromsom 1.
