Die Brachydaktylie vom Typ A4 ist eine autosomal dominante Erkrankung, deren genetische Ursache noch nicht geklärt ist. Charakteristisch ist die Verkürzung der Mittelphalangen, eine Nageldysplasie und eine duplikation der distalen Phalanx des Daumens. Neben dem Kleinwuchs sind weitere Skelettabnormalitäten zu beobachten.
