Hypoparathyreoidismus ist die inadäquate Parathormon (PTH) Sekretion der Nebenschilddrüse.
Die Klinik wird beim primären Hypoparathyreoidismus von den Schäden der Hypocalciämie (Osteoporose, Tetanie) dominiert.
Die Diagnose kann anhand des niedrigen Calcium- und erhöhtem Phosphat-Spiegels gestellt werden, wenn gleichzeitig ein deutlich erniedrigtes Parathormon gefunden wird.
Es wird ein primärer und sekundärer Hypoparathyreoidismus unterschieden. Bei ersterem findet sich eine Hypocalciämie, bei letzterem erhöhte Calciumwerte. Weiterhin gibt es neben den genetischen auch im Laufe des Lebens erworbene Formen.
Der familiäre Hypoparathyreoidismus kann durch eine funktionssteigernde Mutation im Gen des Calcium-Sensing-Rezeptors hervorgerufen sein. Wegen der überschießenden Rezeptorfunktion erfährt die Epithelzelle der Nebenschilddrüsen nicht von der Hypocalzämie und kann deshalb nicht mit einer adäquaten Parathormon-Sekretion antworten.
Die Symptome können durch die Substitution durch Parathormon und Calcium gebessert werden.
